Team Approach

We want to create a partnership with parents, principals, doctors, teachers, nurses, and other school support staff to ensure the safest possible environment for students. By working together, we can create a plan that can help meet your student’s special dietary needs.

Special Diets

If your child has a medical dietary need that requires food to be different or to be modified, please contact us by phone or email at specialdiets@laveeneld.org. Have the Special Diet form filled out and signed by your doctor. Please feel free to call our office at 602-237-9100 ext 3043 if you have any questions.

If the special diet request is related to religious or other non-medical preferences*, please contact your cafeteria manager to discuss how we can tailor our daily choices for your child. You may also visit laveen.nutrislice.com to review nutrition information, filter our menus by allergens, and view carbohydrate counts.

*Laveen Elementary District School cafeterias are all peanut-free facilities. We do our best to avoid serving pork products. A vegetarian option is available on a daily basis.

This may include Diabetes, Celiac Disease, Swallowing Disorders or other disabilities that may affect a student’s diet. Please notify your school nurses and fill out the appropriate form and send to specialdiets@laveeneld.org or turn into your School Health Associate’s office.
